Why Do Pakistanis Get So Much Acne? Climate, Hard Water, and Heat

The most common assumption about acne in Pakistan is that it is genetic — that South Asian skin is inherently more acne-prone than other skin types. This assumption leads to resignation: acne is simply how Pakistani skin is, something to be managed rather than genuinely cleared.

The reality is different. Pakistani skin is not inherently more acne-prone. It is operating in an environment with four specific factors that create the conditions for persistent, severe acne more reliably than genetics alone. The same skin type, in a cooler, softer-water, lower-pollution environment, would develop significantly less acne. Understanding these four factors is the starting point for understanding why standard acne treatments often feel insufficient for Pakistani skin — and why Pakistan-specific approaches produce better results.

Factor 1: Heat Drives Excess Sebum Production

Sebaceous glands produce more oil at higher temperatures. This is a physiological fact, not a skin type characteristic — the same glands that produce moderate oil in cooler conditions produce significantly more in Pakistan's sustained summer heat. Above approximately 30°C, sebum excretion rate measurably increases. Pakistan's climate maintains temperatures above 30°C for eight to nine months across most of the country.

The consequence for acne: the primary raw material of acne formation — sebum inside pores — is continuously produced at elevated levels. Pores that might take two to three days to develop a meaningful blockage in cooler conditions develop one overnight in Pakistan's heat. The cycle of comedone formation, inflammation, and breakout runs faster in Pakistan's climate than the same skin would experience elsewhere.

This is why many Pakistanis who travel abroad or move to cooler climates notice significant acne improvement without changing any skincare products. The environmental sebum stimulus reduces, and breakout frequency reduces proportionally.

Factor 2: Hard Water Traps Dead Cells in Pores

Pakistan's water supply across most cities and towns is classified as hard water — high in dissolved calcium and magnesium. These minerals do not harm skin to drink but actively worsen acne formation at the skin surface.

When hard water dries on the skin after washing, the dissolved minerals remain as deposits. These deposits have two direct effects on acne:

They disrupt the skin surface pH. The skin's natural pH is slightly acidic (4.5 to 5.5). Hard water minerals alkalise the skin surface above this range after every wash. In the alkaline environment, the enzymatic processes that regulate natural dead cell shedding from pores are impaired — dead cells remain in pores longer than they should, providing more material for the sebum-dead cell blockages that create acne lesions.

They impair the skin's natural exfoliation. Dead cells that would naturally shed from pore walls in soft water conditions stick longer in hard water conditions, because the mineral deposits interfere with the enzyme activity that loosens them. More dead cells staying longer in a high-sebum environment is exactly the formula for accelerated comedone formation.

This hard water effect explains why Pakistani skin can develop significant blackheads and congestion even with consistent cleansing — cleansing removes surface oil but cannot address the mineral deposits and pH disruption that slow natural pore clearance between washes.

Factor 3: Pollution Adds a Third Layer of Congestion

Urban Pakistan carries some of the highest airborne particulate matter concentrations in the world. Lahore's air quality regularly ranks among the worst globally during winter smog seasons. Karachi and Islamabad face significant pollution from traffic and industrial sources year-round.

Particulate matter from pollution settles on the skin continuously. These particles are small enough to enter pore openings, where they mix with sebum and dead cells to form a congestion layer that is more resistant to normal cleansing than sebum alone. The tripartite combination — excess heat-driven sebum, hard-water-retained dead cells, and pollution particles — creates pore blockages that are denser and more established than any single factor would produce alone.

This pollution-sebum-dead cell combination is why Lahore and Karachi residents frequently experience more stubborn congestion and blackheads in the nose and forehead area than the same skin type would experience in lower-pollution environments.

Factor 4: UV Radiation Prolongs Every Mark

Pakistan's UV index of 8 to 11 does not cause acne directly — but it dramatically extends the visible duration of every acne event. Post-inflammatory hyperpigmentation — the flat dark mark that remains after a pimple heals — is worsened by UV exposure. Every day of unprotected sun exposure triggers melanin production in the recently inflamed area, darkening the mark and extending how long it remains visible.

In lower-UV markets, a post-acne mark might fade in six to eight weeks. In Pakistan's conditions, the same mark can persist for four to six months or longer — because each day of unprotected UV exposure refreshes the melanin trigger before the mark has time to fade through normal cell turnover.

This UV factor is why Pakistani acne appears more severe than it may actually be — breakout frequency may be decreasing but the marks from previous breakouts remain dark and visible, creating the appearance of continuous active acne.

What This Means for Treatment

Pakistan-specific acne requires Pakistan-specific treatment priorities:

Address the sebum overproduction: Salicylic acid serum to clear existing pore blockages. Niacinamide to reduce the production that creates new ones.

Counteract hard water: Gentle cleansers that do not compound the hard water pH disruption. Chemical exfoliation that dissolves the dead cells hard water keeps in pores.

Protect against UV deepening marks: SPF 50 every morning — not optional in Pakistan's conditions. Without SPF, UV darkens every existing mark while treatment works to fade it.
